I'm trying to detect uninstalls of our iOS app.
I read this document which gives some useful information. However, how can I understand from the error codes returned whether the user uninstalled versus disabled push notifications/has no connectivity?
I think you'll want to use the Feedback Service. When a user deletes an app, the service provider should ideally stop sending notifications to that device. But Apple does not notify the service that "this device is not using your app, dont send notifications". Technically, a device which has uninstalled your app will not make it onto this list until the next time a push notification goes to the device. So you need to poll for this info using the Feedback Service.
Periodically, you will need to hit Apple Notification servers asking it to give you IDs that have deleted your app. Once you get them you mark them in your DB as deleted thereby not sending any more notifications. This Feedback Service will tell devices that have been unregistered (app uninstalled). The part I'm not sure about is whether a user who has turned off push notifications in settings will register the same. I believe they will not show up in the feed from the Feedback Service. I am certain, however, that users who are offline and the push notification is not delivered will not be included in the list.
It would be a simple test in your dev region to try the app, disable push notifications for the app, and then see if the device shows up in the feed.

I am doing MDM implementation in iOS and I have one server for its implementation.I have gone through the documents and understood the process that we have to undergo.But Simply I don’t understand how it will happen.I have one server and one device.I have my PEM file enrolled in the server.
As the process of MDM says server sends push notification through the device and the information that is present in the message in only the identifying token.Then device is connected to the server and sends an idle message confirming that device is ready for the commands.
My question is how device receives the push notification.If app is in back ground state and it gets a notification to connect to the server is there any method to detect it and send the feedback or is it happening automatically or we have to write some code for it. Also how does the plist (that we get in command) make changes in the settings (unlock to lock) .Do we save it somewhere in the device configuration?
I have got a lot of questions about the command format but did not get anything how the command works.May be I am missing basic iOS coding.please just explain me the above format.

First of all, you will have to implement server side only for iOS MDM. Client side is implemented by Apple and MDM client is baked into iOS.
As part of OS it dosn't have restriction which usual iOS apps has. It has a persistent channel to APNS and as soon as somebody send a push notification to APNS, the device will receive it through this persistent channel and will start doing what it should be doing.

The answer is the same. You don't have to implement anything on the iOS side. It's all done by Apple- MDM client will get the command, parse it, save it to appropriate places and enforce appropriate policies.
The documentation specifically states that the feedback service will report devices where the app has been uninstalled and won't report devices if the push merely expired.
My question is, will it report devices that have the app installed but the app called unregisterForRemoteNotifications (e.g. the user "signed out" of the app)?

The Apple Push Notification Service includes a feedback service to give you information about failed push notifications. When a push notification cannot be delivered because the intended app does not exist on the device, the feedback service adds that device’s token to its list. Push notifications that expire before being delivered are not considered a failed delivery and don’t impact the feedback service.

My answer is YES, APNS does have this service in place along with its feedback service, mainly because the server triggering the push service will have to be notified back to stop sending push notification to the devices which have manually opted out of push notification service.
And since there is no way this information can reach back to the server directly except through APNS, APNS manages to hold this information to provide it back to the server.
How do iOS "push" notifications get delivered to a particular device without that device needing to poll a server?
For example, let's say I have received a new message on Facebook. Facebook notifies Apple that my device should receive a notification as such. But how does Apple know which device/IP to push the message to?
Each device can be updated with data using their own unique device tokens. This picture explains everything . .

Each device establishes an accredited and encrypted IP connection with the service and receives notifications over this persistent connection. When new data for an application arrives, the provider prepares and sends a notification through the channel to APNs, which pushes the notification to the target device..

Push Notifications

Device does not keep polling the server for the push notifications.
To keep it simple, consider an iPhone is connected to internet. On connecting to internet iPhone establishes connection to Apple Push Notifications server this connection is open connection which means data can be thrown to iPhone from server the moment data arrives to server.
Apple does not use HTTP protocol for Push notifications but if you understand HTTP Protocol its almost a similar methodology.

In iOS, apps can’t do a lot in the background. Apps are only allowed to do limited set of activities so battery life is conserved.
But what if something interesting happens and you wish to let the user know about this, even if they’re not currently using your app?
I'm developing an iOS app that enables the user to observe technical devices and be notified if there are problems.
What I know so far:
My app requests a token from the APNs.
My app sends this token to my server.
My server notifies the APNs in case of an event.
The APNs pushes a message onto the device.
What I want to know:
I read the token may change, therefor I need to request it on every app launch. Is this true?
If I get a new token from time to time I have to register at my server from time to time to make sure it uses the current token. Do I have to store and manually send the old token with the new one if the token changes to allow my server to delete the old one?
How can I detect that a user removed the app to remove his device on the server?
Highest priority in my case is for the server to know which devices are registered with the service. Old devices (old tokens that is) need to be removed immediately.

You should register for the remote notification on every app launch and send the token to your server.
To check if the user removed the app or disabled notifications you have to check the feedback service. Look for it on this page:

Note: APNs monitors providers for their diligence in checking the feedback service and refraining from sending push notifications to nonexistent applications on devices.
I implemented push notification in my app.
It is working fine.
Now the problem is even after i deleted my app from device it is getting the push notifications.
So is there any way to unregister the app from push notification when it is deleted from the device.

Having not seen this answer so far, there is a small note in the Apple "Troubleshooting Push Notifications" document.
In short, if you delete the last push enabled app, the persistent connection from the device to Apples push server is broken before the server is told that the app has been deleted.
Solution: keep at least one push enabled app on your device.

Issues with Using the Feedback Service
If you remove your app from your device or computer and then send a push notification to it, you would expect to have the device token rejected, and the invalidated device token should appear on the feedback service. However, if this was the last push-enabled app on the device or computer, it will not show up in the feedback service. This is because deleting the last app tears down the persistent connection to the push service before the notice of the deletion can be sent.
You can work around this by leaving at least one push-enabled app on the device or computer in order to keep the persistent connection up. To keep the persistent connection to the production environment up, just install any free push-enabled app from the App Store and you should then be able to delete your app and see it appear in the feedback service.
Recall that each push environment has its own persistent connection. So to keep the persistent connection to the sandbox environment up, install another development push-enabled app."
