Is there anyway to get notified when a user posts a review of your app in the app store? E.g. it would be nice to get an email every time a new review of my app is posted.

It seems as though you do not want to log into itunesconnect and look for reviews every day. Try signing up for a service like appannie.com It is free and they send you a daily email with downloads from the previous day and any new reviews that you have received.

I want to make an app through which patients can consult doctors through video calling and would be charged by the doctor based on the duration of the call.
Will apple take a share from each transaction?
Apple only takes its cut from the purchase of paid apps and the purchase of in-app purchases. They charge 30% of what each user pays to them.
You can integrate your own payment methods for calls etc if you want to charge user for each call. So, there would be no involvement of Apple in this transaction.

iam doing the one application.I want to set the monthly subscription for that application. Means I want to get the transaction receipt when I install the application on device. And every month user wants to renewal the subscription. If not renewal application not open.This is my requirement . So please tell me how can apply the subscription for total application .
You can't make the app to not open if third-party subscription ends.
That would look like a crash.
What you should make is when a user opens your app you tell him to renew the subscription and how.

I would like to ask a question about submit process of an iOS application. The application which I would to submit has a private area accessible by credentials. Have I to send the credentials to Apple for testing?

When you submit the app, there is a field near the bottom of screen saying Review Notes . You can add the test credentials there, which will be noticed by the reviewers at Apple.
Yes, you should give Apple the credentials. There is box in iTunes Connect where you can give Apple the extra information needed to test the app.

I have tested in app purchase for a long time.
For testing in app purchase I have use TEST USER that I have crated in iTunes Conect.
But now when I try to sign in with my TEST USER account I get this message (before this message not appear)
So, this means that I have to enter all info about myself (bank account and etc). But I think for test user this items not needed.

This often happens if you accidentally attempt to purchase real content with a test user. Try defining a new test user and deleting the old one.

I just uploaded an app that uses an external restaurant booking provider. This provider has only enabled one restaurant for testing purposes. Now how do I include this information to apple reviewers so they can test using this restaurant?
The provider will only enable all the restaurants for booking once the app is live.

When you upload either a new app or an update to an app using the interface in iTunes Connect, there is a section as part of this process called Review Notes.
This is a regular textbox and is the right place for you to post this information.
