It there a visual db editor (similar to Navicat - http://www.navicat.com/products) that is designed to work with rails migrations and other rails-like approaches to models?
Specifically, I would like a graphical tool that can aid in the creation and maintenance of rails models/migrations.

I've never seen a visual tool to actually edit or design migrations. But there is one to draw a graph of an existing setup. It includes has_many relationships which are defined in the model. It is called Rails ERD (Entity-Relationship Diagrams for Ruby on Rails):

I've searched github and just can't find a good example of a simple crud app that meets the following criteria:
Rails 4
simple crud operations type app (a todo list, contact manager, a blog/cms, etc)
responsive design (preferably Bootstrap 3)
AJAX crud operations
a live demo url (preferably on heroku)
clean, simple, and slick
updated repo in the last 6 months
Anyone care to share?

I think the code sample in the book “Agile Web Development with Rails” is great. Just go with it.

is there any rails tutorials that teaches without the use of rake commands or other rails commands that result in code being auto generated for you or other luxuries that favour productivity over understanding.
If you have trouble following, for example http://ruby.railstutorial.org/book/ruby-on-rails-tutorial (which is basically THE TUTORIAL for Rails) it is very likely that you have trouble with Ruby and not with Rails.
I would suggest that you dig (deeper) into Ruby (codecademy.com has a very nice Ruby branch) and Rails becomes much easier to understand.
At least that was my learning experience!
On the code generation:
It makes sense to generate these stubs because they adhere to the MVC paradigm. If you don't understand that part, do some research on that.

How are people using it etc.
I know the people at pivotal use Selenium, I'm looking for an insiders view on how to use it effectively and efficiently etc.

Are any alternatives to the Active Record in Rails?
Yes it is possible but not very easy.
In rails 3 the ORM layer will be pluggable and thus plugging in another ORM layer will be easier.
Datamapper is the biggest alternative ORM but there are others.
I steer clear because when you use these you really end up on your own out on the bleeding edge and as yet I haven't a need that justified the risk.
