Why web application running on phusion passenger is getting down frequently? - ruby-on-rails

My web application running on phusion passenger is getting down frequently but i am not able to trace out the reason.
Here is the error i got in /var/log/httpd/error.log file

Apache2 Passenger not updating Ruby on Rails classes on restart

Forgive me as I am a bit new to this but I have inherited an Ruby On Rails web application that runs on Apache2 Passenger. I have tried to make a small change to the application which works succssefully on DEV so I have deployed to our PROD environment. However after deploying and restarting Apache2 none of my changes are taking effect. If I change the
config.cache_classes = true
Setting in the config/environments/production.rb to false then all is fine, however everything I read is that should only be set to false in dev.
This leads me to believe that the cache is not being refreshed once Apache2 is started. Any ideas on what this could be? I don't see any errors in the Apache2 error log (debug level set), below is a snippet from a restart.

Any thoughts would be greatly appreciated.
I managed to figure out the problem, when deploying my coding changes I backed up the original file in the same directory just appending a date to it, for example my personnel_controller.rb was renamed to personnel_controller_20220926.rb and the modified personnel_controller.rb from dev was moved over. For whatever reason Rails (or maybe Apache) was still interrupting the old file rather then the new one. Once I moved the old file to a new location outside of the applicaitons directory and restarted Apache all worked fine. I am still new to Ruby on Rails in general so if anyone has an explanation as to why this is I would be curious to know.

Unicorn gets killed due to out of memory

Today, I have noticed that unicorn was not running on the production environment. Unicorn error log is as follows (it seems unicorn stopped due to SIGKILL signal):

It seeems when the unicorn was killed, we had faced a out of memory problem. In the /var/log/syslog.1 I have following logs:

Now my questions are;
How I can make sure that unicorn will be restarted automatically even if it gets killed by kernel? (eg, restart automatically after 30secs)
In the normal operation, memory utilization is around %50-%60. I would like to understand the root cause and take measures to prevent a similar problem in future. According to the syslog messages, out of memory issue is started with postgres. I have no clue what caused a high memory utilization by postgres or other processes. Any kind of support will be appreciated.

Issue with iOS low memory crash but using very low memory - checked with instruments

I have a sample App that works fine on the simulator, but crashes on the device. I get the following output on the console from Organizer. It crashes within a short time.
Using instruments, memory leak, I see that the memory usage is about 2-3 mb. Using memory allocations, I see around 8-9mb for memory allocations and nothing in VM tracker.

ok, so it seems like everything was fine, I just needed to delete the app from the iPad and reinstall it... pretty weird... !!!

UIWebView strange behavior on different devices

I have developed a sample iPad application in which I open the one file of type ppt in UIWebView. When I run this application on iPad having O.S version 5.0(iPad2) & 4.3.1(iPad1) the application gets crash with following error(Memory Warning):

But when I run same application on iPad having O.S version 4.3.3(iPad2) it runs perfectly. What is the reason behind this strange behavior & any solution to solve this. Thanks in advance.
your app is crashing because it ran out of memory.
Try using PDF format or PPS (power point slide show) format. That may take some lesser space.

Apache shutting down unexpectedly

I have a mongrel server running behind Apache. It works fine; however, every now and then the Apache server shuts downs seemingly by itself. I'm not sure if there is configuration issue or if it's an attack. Here is Apache error log:

Not quite sure what is /var/www/fastenv but I don't think there is anything in my application that calls that. Also, website is still in Beta mode with few users and I don't think any have 61.139.105.163 IP address but it's possible that they might have it.
Any ideas? It would be good if you can give me hints where to look or how to go about anaysing this problem
I have the exact same log from the same IP. Looking it up shows it to belong to the Chinese government. It appears to be a scan using server side includes to find out as much as they can about your server. I banned the IP.
Not sure this is entirely programming-related, but anyway... none of those look like serious errors to me. The accesses to /var/www/fastenv just mean that the computer at IP address 61.139.105.163 sent a request for http://www.example.com/fastenv or something like that (it depends on exactly how you've configured your virtual hosts); I'd look at the access log for more information, to see what other requests have been coming from that IP address. It's probably not anything to worry about.
The line about NameVirtualHost *:0 means that somewhere in your configuration file you have an incorrect NameVirtualHost directive, maybe with no arguments. You should probably look for that and remove it, but if the server is running fine anyway, it's not a big deal.
The reason your server is terminating (restarting, actually) appears to be a SIGHUP - that is, something on the system is sending Apache a signal telling it to restart. It's basically the same thing that happens if you run apache2 restart, I think. Without knowing what's sending that signal, there's not more I can say.
61.139.105.163 is known for doing all kinds of hacking type things, just google the IP address. You should definitly ban this IP address.
Click on Apache Config --> Apache(httpd.conf)
Search for #Listen 12.34.56.78:80 and replace it with #Listen 12.34.56.78:8081.
Search for Listen 80 and replace it with Listen 8081.
Now you can start Apache now, and can run it with this URL: localhost:8081/xampp/
