I have access to the HL7 Clinical Document Architecture, Release 2.0, which states that it is used essentially to link entries with each other in a CDA document. Specifically, it links between what is called the "source" and the "target" entries. I also read about the different types of relationships (CAUS, COMP, GEVL, MFST, REFR, RSON, SAS, SPRT, SUBJ, XCRPT) and somewhat understand those.
My main question: what are the "source" and "target" elements? Are they the element containing the entryRelationship, and the element contained by entryRelationship?
For example:

In the above snippet, according to my understanding, there is a SUBJ relationship between the act the the first observation, and there is a REFR relationship between the two observations. Is this correct?
The source of a entryRelationship is the entry that contains in its body the element in your example the source entry is
<act classCode="ACT" moodCode="EVN">
and the target is
<observation classCode="OBS" moodCode="EVN">
Its is posible to indicate a inverse relationship using the "InversionInd" attribute of the entryRelationship elemet. If this attribute is set to true source and target are inverted.

In inherited invoice_report xml document i have conditional field that needs to be shown - if column (field) in db is equal to another column. To be more precise - if invoice_origin from account_move is equal to name in sale_order.

You almost had the solution, however text() is used to reference the inner text of an XML element, such as <element>This text here</element>. Here is the syntax for referencing an attribute's value:

Xpath is query language for XML, and is very flexible.
To get you started:
doc = Nokogiri::XML::Document.parse( xml_string )
parts_from_abc = doc.xpath( '/parts/part[madein/city="ABC"]' )

Just use:

Remember:
In XPath 2.0 (XSLT 2.0) use the eq operator for value comparissons -- it is more efficient than the general comparisson operator = which really, only, needs to be used when at least one of its operands is a sequence.
I would try this:
<xsl:value-of select="course/department[#code=$dep]/dept_short_name/text()"/>
That says: find the department element (inside a course element) whose code attribute is the value of parameter "dep", then find the dept_short_name child element, then get the text inside that element.
You have to use the # to say that "code" is an attribute, but "dep" should not have it. I think the {} notation is for use inside attributes of the non-XSLT elements of your stylesheet, so I wouldn't use it inside a value-of expression.

Here's an example (basically digest from Neil's links):
Your XUL file:

This works for small numbers of entities. For instance, menuitems sometimes have two entities with slightly different text depending on what the menuitem will be used for, and the correct entity is then copied to the label. The worst abuse of this was for the Delete menuitem in Thunderbird and SeaMonkey's mail windows, which had labels for unsubscribing from newsgroups, deleting folders, cancelling news posts, deleting single or multiple messages, or undeleting single or multiple messages from folders using the IMAP mark as delete model. Phew!
If you have lots of locale data then the best thing is to put it in its own .properties file and read it using a <stringbundle>. If your script doesn't have access to a <stringbundle> element it is also possible to manually retrieve an nsIStringBundle from the nsIStringBundleService.
